To Our New Patients:

Welcome to Palm Beach Fertility Center!

Our friendly staff is here to address your concerns and answer your questions. We realize that your time is valuable therefore we ask that you bring the following items with you to your initial consultation:

All pertinent medical records, especially prior fertility treatment cycles.

• All operative reports.
• Insurance authorization if applicable (required for all HMO insurances).

After reviewing your medical records, Dr. Denker will meet with you to discuss your treatment goals and objectives. He will then formulate an individualized treatment plan to help you achieve your goals and make your dream become a reality. He will do everything possible to expedite your testing and treatment.

Our friendly nurses will assist you in making all arrangements necessary to complete any additional diagnostic testing needed, and implement your treatment plan in an efficient manner.

Our office manager Eliza will meet with you to discuss all financial aspects of your treatment plan and will answer any questions related to insurance and financing.

We welcome you to Palm Beach Fertility Center, where we will provide personal care and be sure that all your needs are met. Please do not hesitate to let us know if there is anything you need. We are here to help you.
